Any appointment request or scheduling submission made through the Site is a request only and is not guaranteed until confirmed by our staff.

If you submit referral or patient information through the Site, you agree to provide accurate, current, and complete information. You also agree not to impersonate any person or misrepresent your affiliation with any individual or entity.

4.3 Sensitive Information

Please submit only the information requested by the form. While we take reasonable steps to protect information, no website transmission can be guaranteed 100% secure. If you prefer, you may contact us by phone or fax instead.

4.4 Insurance and Coverage

Insurance acceptance information on the Site is provided for convenience and may change. Coverage and eligibility are determined by your plan and payer rules. You are responsible for verifying benefits, copays/coinsurance, deductibles, authorizations, and any non-covered services.
